How Do I Deal With Super Sun-damaged Skin?

School on Saturday started off really slow. It was taking me awhile to feel fully awake, so I was happy to have some down time. We did get a few clients and pretty soon I was next on the rotation. My client’s age was unknown to me. I think she appeared to be older than she really was because of her skin condition. She had a loss of elasticity, quite a bit of sun damage, and lots of fine lines and wrinkles. I gave her a facial using our organic product line for mature, dry, aging skin. I also incorporated an AHA treatment into the facial. She has had the AHA treatment before, so she knew that her skin would start to tingle and get very warm. I think with regular treatments, she would really notice her skin tightening and firming, and it would even out her skin tone, as well.

Her skin felt different to me than any other skin I have dealt with: it was rather thick and almost leathery. I recommended she use a good firming moisturizer during the day with sunscreen, and also a good night cream. Regular exfoliation would be beneficial for her skin type, and I told her she should really consider coming in more frequently for this particular facial so we can work on her areas of concern. I told her some of our clients come in once a week for facials. But I think a facial once every 2 to 3 weeks might be good for her. Does this seem appropriate for her mature, sun damaged skin type?

    My experience with sun damaged, thick skin is that it needs resurfacing. What the client needs is services and products that create cell turnover and products that have lightening properties to it. A series of peels would be your best bet. Depending on her exact skin type, peels with TCA. Start out with a low percentage like a 6% and if it has lightening properties in it such as L-Arbutin, Azelaic, Kojic Acid that would be a great blend. You want to build their skin up to a higher percentage. They will peel but that kind of skin needs to. At home care should consist of a vitamin C cream or serum and a retinol for the ultimate resurfacing effect. Microdermabrasion would be a good addition if there is no serious vascularity going on, to combine with the peel just do one pass beforehand. How often you can do this treatment is every 3 weeks. She also should wear broad spectrum sunscreen at all times. If you can do this for your client you CAN absolutely change her skin especially her radiance and tone. She will love you forever!

    Monika - Thanks for all the great information. I want to follow your advice and give this woman the great skin that she wants! I will try a peel on her next time and try to incorporate the microderm in there sometime as well. It sounds like you really know your stuff!!! Thanks again!

    Colleen,
    If you are using AHA peel, do the microdermabrasion after the peel. It will be more comfortable for your client. May I presume she’s NOT going out in the sun anymore? Make sure she does not! She has to understand she got herself into that mess by doing just that. Ask her to come for a peel as often as once a week (again with AHA’s). Combine with microdermabrasion every other week. Make sure she uses a daily exfoliator (10% glycolic or lactic), a good moisturizer and sunscreen during the day. Good luck!

    I would highly recommend the Obagi Nu Derm. If you are not familiar with it, it is an amazing at home product that is physician grade. AMAZING results. I have been an aesthetician for about 4 years now and prior to doing a career change I was a product junkie. In my opinion, there is nothing like it. It is a totaly committment because you are red and peeling for about 8 weeks. It actually transforms the skin a cellular level unlike most products they mask the skin and have minimal penetration.
